Alternate Supervisor Application

The alternate supervisor application is for supervisors who are not CACFT approved supervisors (RMFT-SQ, RMFT-S, and RMFT-SM), but may qualify to provide systemic supervision on a case by case basis. Supervisors who are AAMFT approved do not need to complete the alternate supervisor form as they are considered equivalent to RMFT-S..
